Job Summary
A company that provide home health benefits management services is filling a position for a Remote Home Health Licensed Nurse Coach.
Core Responsibilities Include:
- Managing the transition of assigned Post-Acute Care patients from acute care setting to the home setting
- Educating, coaching and influencing patient and caregiver behavior to achieve care plan goals
- Documenting all interactions, guidance and interventions in our applications
Applicants must meet the following qualifications:
- Unrestricted RN licensure in the state(s) of practice
- Associate's, Bachelor's Degree, or higher in Nursing
- RN with a minimum of 5 years' experience
- Excellent negotiation, communication, problem solving and decision making skills
- Excellent organizational and interpersonal skills
- Knowledge of MS Outlook and Excel documents
