Job Summary

A contract therapy company is searching for a person to fill their position for a Remote Home Health LVN Case Manager in Simi Valley.

Individual must be able to fulfill the following responsibilities:

• Managing our home health patient census
• Overseeing and communicating with a large staff of physical, occupational, and speech therapists
• Providing reports to administrative staff and business owner

Required Skills:

• After the training period has been completed this will be a remote work opportunity
• 1 year home health case manager or equivalent position experience
• LVN License
• Must have good basic telephone and computer skills
