Job Summary
A health insurance company is searching for a person to fill their position for a Remote Home Health RN Care Manager in North Jupiter.
Core Responsibilities of this position include:
- Managing a member's physical, environmental, and psycho-social health issues
- Identifying and resolving barriers that hinder effective care
- Ensuring patient is progressing towards desired outcomes by continuously monitoring patient care
Applicants must meet the following qualifications:
- Ability to travel to member's residence within 30 to 40 miles
- Registered Nurse with 2 years of experience of in home case/care management
- Experience working with the adult population
- Knowledge of community health and social service agencies and additional community resources
- Associates working in the State of Florida will need ACHA Level II Background clearance
- All other requirements necessary for this position
