Job Summary

A health insurance company is filling a position for a Remote Home Visit Field Care Nurse Manager 2 in Columbiana County.

Must be able to:

  • Manage a member's physical, environmental, and psycho-social health issues
  • Identify and resolve barriers that hinder effective care
  • Ensure patient is progressing towards desired outcomes by continuously monitoring patient care

Applicants must meet the following qualifications:

  • Ability to travel to member's residence within 30 to 40 miles with valid driver's license and car insurance
  • Registered Nurse with 2 years of experience of in home case/care management
  • Experience working with the adult population
  • Knowledge of community health and social service agencies and additional community resources
  • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel
  • Excellent keyboard and web navigation skills
