Job Summary
A healthcare company is in need of a Remote Home Visits Nurse Practitioner and Physician Assistant in the West Los Angeles Area.
Individual must be able to fulfill the following responsibilities:
- Perform comprehensive initial assessments for newly enrolled patients
- Diagnose, prescribe and treat patients, consistent with standard of care
- Participate in a rotated on-call program for home-based patient support after-hours
Applicants must meet the following qualifications:
- Access to reliable transportation to conduct patient visits; willingness to conduct house calls
- Completion of Nurse Practitioner (NP) or Physician Assistant (PA-C) accredited program
- Current unrestricted NP or PA state license, and current state driver’s license
- Knowledge of clinical standards of care
- Passion for delivering care to patients with complex, chronic illnesses
