Job Summary
A senior living facility needs applicants for an opening for a Remote Hospice Care Coordinator Registered Nurse.
Individual must be able to fulfill the following responsibilities:
- Participating in home health case conferences or other meetings
- Establishing program goals with leadership/executive team sponsor
- Coordinating joint patient visits with home health case managers
Applicants must meet the following qualifications:
- Ability to travel as needed
- Registered Nurse or Licensed Practical/Vocational Nurse
- Hospice experienced clinician with extensive hospice benefit and eligibility knowledge
- High level knowledge of home health processes and practices
