Job Summary
A healthcare company is filling a position for a Remote Hospice RN Case Manager in Cedar Rapids.
Individual must be able to fulfill the following responsibilities:
- Ensure compassionate, empathetic quality and safe delivery of services
- Consistently utilize a patient-centered approach
- Make visits to the homes of patients and assumes responsibility
Must meet the following requirements for consideration:
- Travel is required
- Ability to facilitate the interdisciplinary plan of care
- Delivers quality care consistent with Hospice Conditions of Participation
- Basic knowledge of current, frequently used medications
- 2+ years nursing experience preferred
- 2+ years nursing experience preferred
