Job Summary

A health insurance company is filling a position for a Remote Kidney Resource Services Telephonic RN Case Manager.

Individual must be able to fulfill the following responsibilities:

• Coordinate care for members and services as needed
• Identify gaps or barriers in treatment plans
• Make outbound calls to assess members' current health status

Qualifications Include:

• Multiple state licensures (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses
• Some evening hours where you will be expected to work until approximately 8 pm
• Active, unrestricted RN license in your state of residence
• 2 years of RN experience in a hospital, acute care, or direct care setting, or experience as a Telephonic Case Manager
