A health and wellness planning company has an open position for a Remote Lead Registered Nurse Care Manager.

Core Responsibilities Include:

• Managing inbound member inquiries from our online messaging platform
• Proactively ensuring clinicians are prepared for their member visits each day with all necessary patient information
• Closing gaps in care for patients and coordinate appointments with specialists

Qualifications Include:

• Active Registered Nurse License
• Excellent oral and written communication skills
• 2+ years in healthcare, operations, clinical care, and Bachelor's degree in Nursing from an accredited college or university
• Strong leadership capabilities with a passion for developing your team
• Solid analytical skills, sound judgment, and the ability to problem-solve
