Job Summary
A hospital and healthcare technology company is seeking a Remote Lead Social Worker.
Individual must be able to fulfill the following responsibilities:
- Collaborate and coordinate with the payer to help connect patients with medical care
- Maintain timely patient medical records
Applicants must meet the following qualifications:
- 3 - 5 years of experience providing social work/case management services
- Experience and proficiency working with an Electronic Medical Record system
