Job Summary

An insurance agency is searching for a person to fill their position for a Remote Licensed Behavioral Health Case Manager I in the Buffalo Area.

Candidates will be responsible for the following:

  • Monitoring and evaluating effectiveness of care plan and modifies plan as needed
  • Conducting assessments to identify individual needs and develops care plan
  • Using appropriate screening criteria knowledge and clinical judgment to assess member needs

Required Skills:

  • Attend occasional dept. meetings.
  • 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse
  • Requires MS or MA in social work, counseling, or a related behavioral health field or a degree in nursing
  • Licensed in NY state
  • Experience in case management and telephonic and/or in person coaching with members
  • Knowledge and experience in Children's health and BH services, HCBS, EBPs, EPSDT services and social service programs
