Job Summary
A healthcare company is seeking a Remote Licensed Care Coordinator.
Must be able to:
- Provide care coordination to members with behavioral health conditions
- Conduct in depth health risk assessment and/or comprehensive needs assessment
- Serve as point of contact to ensure services are rendered appropriately
Position Requirements Include:
- Valid driver's license
- Master's level independently licensed behavioral health professional
- 5 plus years post degree behavioral health experience
- Ability to be credentialed for independently licensed behavioral health professional
- Solid understanding of behavioral health diagnoses
- Knowledge of community resources
