Job Summary
A healthcare provider is in need of a Remote Licensed Chronic Care Management LPN.
Must be able to:
- Coordinate care for enrolled patients including scheduling appointments
- Provide patient education to assist with self-management
Must meet the following requirements for consideration:
- 3 years of experience as a Care/Case Manager
- Strong working knowledge of chronic disease states
- Unrestricted and current license to practice as an LVN/LPN/RN with a compact license
