Job Summary
A specialty healthcare company is seeking a Remote Licensed Clinical Social Worker in Rochester.
Core Responsibilities of this position include:
- Participating in care planning in collaboration with patient & healthcare team to identify effective interventions that will help the patient
- Assessesing patient knowledge of kidney disease for barriers that may affect adherence to treatment
- Assessing for social determinants of health that may impact treatment & assisting with identifying community resources
Applicants must meet the following qualifications:
- Master’s degree in social work
- Active and unrestricted state-specific licensure required; Licensed Clinical Social Worker
- Unrestricted driver’s license and reliable transportation
