Job Summary

A specialty healthcare company needs applicants for an opening for a Remote Licensed Clinical Social Worker in Syracuse.

Must be able to:

  • Assess patient knowledge of kidney disease for barriers that may affect adherence to treatment
  • Participate in care planning in collaboration with the patient and healthcare team
  • Assist patient and/or family/caregiver in completing applications for services and/or resources

Required Skills:

  • May require to see local patients if they live in their radius for patient home visits
