Job Summary

Remote Licensed Master Social Worker, this full-time position involves providing virtual support to patients facing complex medical, social, and behavioral health challenges, while collaborating with an interdisciplinary care team to improve patient outcomes.

Key Responsibilities

• Collaborate with interdisciplinary team members to deliver comprehensive virtual care services

Required Qualifications

• Master's degree in Social Work from an accredited program
• Active unrestricted Social Worker license in the state of residence
• 1+ years of experience in social work, preferably in a healthcare setting
• 2+ years' experience in care delivery to patients with chronic diseases
• Knowledge of healthcare regulations and commitment to patient confidentiality
