Job Summary

A company is looking for a Remote Licensed Practical Nurse - LPN - LVN - Intake Coordinator.

Key Responsibilities

• Conduct preliminary health assessments for newly enrolled patients
• Resolve patient inquiries and identify community resources for care coordination
• Provide patient education on chronic condition management and assist with medication management

Required Qualifications, Training, and Education

• Active Compact/Multi-State license as an LPN or LVN
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a fast-paced environment
• Must have a home office or HIPAA-compliant workspace
