Job Summary

A company is looking for a Remote Licensed Practical Nurse - Eastern Time.

Key Responsibilities

• Conduct monthly clinical check-ins and engage with patients through outreach
• Perform clinical assessments and escalate urgent issues as needed
• Document patient interactions and maintain compliance with regulatory standards

Required Qualifications

• Active LPN or LVN license
• 1-2 years of clinical experience in ambulatory care, population health, or care coordination
• Ability to work with EHR and CCM platforms simultaneously
• Intermediate skills with MS Office Suite
• Must have a dedicated workspace that ensures privacy for patient information
