Remote Licensed Practical Nurse

Job Summary

Working remotely in the Mountain time zone, the part-time Licensed Practical Nurse will conduct monthly clinical check-ins, manage patient engagement, and ensure accurate clinical documentation while collaborating with care teams to support patients with kidney disease.

Key responsibilities

• Perform monthly patient outreach and engagement through structured assessments and relationship building
• Document patient interactions and maintain compliance with regulatory and internal standards
• Coordinate care by escalating urgent issues and collaborating with providers on symptom management and follow-up

Required qualifications

• Active LPN or LVN license
• 1-2 years of clinical experience in ambulatory care, population health, or care coordination
• Strong clinical assessment and documentation skills
• Intermediate proficiency with MS Office Suite and ability to work in dual systems (EHR + CCM platform)
• Reliable home internet connection with minimum speed requirements and a dedicated workspace to ensure privacy
