Remote Licensed Practical Nurse

Job Summary

Candidates will be responsible for the following:

• Conduct wellness check-ins
• Work closely with LPNs, RNs, and primary care physicians to support patients remotely
• Assist in creating a personalized care plan for each patient

Required Skills:

• Healthcare experience as an LPN/LVN with active license
• At least 6 months in a Call Center environment is a plus
• Case management or care coordination experience is a plus
• Must be able to respond to patients in a professional manner, be quick thinking and able to engage with different types of personalities
• Able to work for 40 hours a week, where you must be speaking to patients on the phone
• Ready to work from home and experience doing so
