Job Summary
A company that provides physicians with custom resources for patient management is filling a position for a Remote Licensed RN Chronic Care Coordinator.
Must be able to:
- Perform telephonic encounters with patients
- Develop a positive interaction with patients
- Develop detailed care plans for the both the doctors and patients
Position Requirements Include:
- Registered Nurse with current active license in state of residency
- Understand health care goals associated with chronic disease management
- Ability to provide prevention and intervention for multiple disease conditions through motivational coaching
- Experience using Electronic Health Records
