Job Summary

A staffing agency is searching for a person to fill their position for a Remote Local Care Coordinator Registered Nurse.

Must be able to:

  • Develop documentation for the Care Plans for any assigned patients
  • Provide on-site consultation to providers and patients
  • Facilitate and monitor the transition of care for assigned patient

Skills and Requirements Include:

  • Ability to travel 50% by own automobile to assigned local practices
  • Active RN License in Maryland AND D.C.
  • 3+ years clinical acute care Nursing experience
  • Utilize databases, spreadsheets, Microsoft office, and other technologies in effective manner
