Job Summary

A staffing agency has an open position for a Remote Long Term Care Coordinator in Gainesville.

Must be able to:

• Complete assessments with members, caregivers, or providers
• Monitor delivery of services and follow-up with members, caregivers, or providers
• Authorize and coordinate referral for services

Skills and Requirements Include:

• Must have valid Florida drivers license
• Must live in Alachua County
• Bachelor’s or RN or LPN and 2+ years of care management experience or Diploma AND 6+ years of  experience
