Job Summary

A company is looking for a Remote Licensed Practical Nurse - LPN - LVN.

Key Responsibilities:

Manage patient census with a resolution-driven approach to close gaps in patient care
Identify and coordinate community resources for patient care
Provide patient education on chronic condition management and medication

Required Qualifications:

Active Multi-State/Compact License (LPN) (NCL) (LVN)
Technical aptitude in Microsoft Office Suite
Experience with Epic software is a plus
Bilingual skills are advantageous
Additional single state licensures (LPN) are beneficial
