Remote LPN Care Coordinator

Job Summary

A company is looking for a Remote Care Coordinator - LPN - LVN for a full-time position.

Key Responsibilities

• Manage patient census to address gaps in clinical and non-clinical care
• Identify and coordinate community resources for patient care
• Provide education on chronic condition management and perform medication management

Required Qualifications

• Active Compact/Multi-State license as an LPN or LVN
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Must have a home office or HIPAA-compliant workspace
• Ability to work in a sedentary position for extended periods
