Job Summary
A platform that provides physicians with custom resources for patient management. is filling a position for a Remote LPN Care Coordinator in Durham.
Must be able to:
- Manage and support patient’s health care needs via phone conversations and via technology
- Manage and support a personalized care plan for each member to address their physical, mental, and preventative health
- Coach members to reach their goals of medication management
Position Requirements Include:
- LPN License
- 1-2 years of clinical experience
- Experience in care coordination or care management, Health Coaching, Nursing, Motivational Interviewing counseling
- Excellent problem-solving skills
