Job Summary
Seeking a motivated and detail-oriented LVN for Chronic Care Management, the full-time remote position will manage high-volume outbound patient calls, conduct comprehensive assessments, and develop care plans while utilizing AI tools to enhance clinical documentation and care coordination.
Key Responsibilities
- Operate in a high-volume clinical call center, managing inbound and outbound patient interactions
- Conduct outbound calls to check on patients, addressing health concerns and ensuring timely outreach
- Document all patient interactions accurately in real time within the clinical platform
Required Qualifications
- Active LPN/LVN Compact License required
- Passionate about patient care with the ability to relate with empathy and compassion
- Proficient with computers, EMRs, and telehealth tools
- Strong communication and organizational skills
- Experience in a call center or high-volume phone-based work environment preferred
