Job Summary
A non-profit managed care organization is in need of a Remote Maternal Child Health Case Manager.
Candidates will be responsible for the following:
- Assessing, evaluating, planning, implementing, and documenting care of members
- Being responsible for the operational delivery of the plan's case management
- Planning care in collaboration with members of the multidisciplinary team
Must meet the following requirements for consideration:
- Ability to present in a group setting
- 3 to 5 years facility-based and/or outpatient psychiatric and substance abuse/substance abuse disorder treatment experience
- Current, unrestricted license in the State of Washington as a Social Worker, Mental Health Counselor, or Marriage and Family Therapist
- Knowledge of Medicare and Medicaid regulations
- Experience in care management workflow systems
