A health insurance provider is in need of a Remote Medical Discharge Planner.

Individual must be able to fulfill the following responsibilities:

  • Manage a continuum of care from admission and two weeks post-discharge for assigned patients
  • Monitor operational key performance indicators to track service delivery against targets

Skills and Requirements Include:

  • Two (2) or more years of healthcare-related experience
  • Awareness and experience with HIPAA requirements for healthcare communication
  • Active, Licensed Practical Nurse (LPN) license
