Job Summary

A major healthcare company is filling a position for a Remote Medical Field Case Manager in Little Rock.

Individual must be able to fulfill the following responsibilities:

• Meeting a members benefit plan and/or health needs in their home or hospital
• Identifying potential barriers to care
• Conducting face to face visits with members enrolled in the case management program

Qualifications Include:

• Percent of Travel Required 10 - 25%
• Nursing/Registered Nurse (RN) license is required
• 5 years clinical practice experience required
• 2 years of Home Health Care/Hospice Care required
• Minimum of three years case management experience
• Excellent analytical and problem solving skills
• Bachelor's degree or equivalent experience
