Job Summary
A healthcare company has an open position for a Remote Medical Therapy Management Pharmacist.
Candidates will be responsible for the following:
- Contacting patients telephonically to review current list of medications on record and ensuring record is up to date
- Discussing problems/issues with medications and making recommendations based on clinical expertise
- Contacting physicians via phone to discuss gaps in care
Applicants must meet the following qualifications:
- Bachelor's Degree in Pharmacy or Doctorate in Pharmacy (PharmD)
- Required experience can be reduced from 3-5 to 1-2 years if the candidate completed a clinical residency program
- 3 - 5 years in Pharmacy
- Registered Pharmacist
- Oral & Written Communication Skills
