Job Summary
A healthcare company focused on primary care is searching for a person to fill their position for a Remote Medicare Advantage Care Management Director in Bethesda.
Individual must be able to fulfill the following responsibilities:
- Manage key care management projects and initiatives to support partner practices succeed in value-based contracts
- Use an evidence-based, data-driven approach to promote care management process fidelity and outcome success in company contracts
- Measure clinical outcomes using internal and external benchmarking to build/modify action plans and deliver improvement
Qualifications Include:
- Willingness to travel as needed to company's headquarters or markets
- Bachelor's degree in nursing
- 3+ years of registered nursing experience in acute, ambulatory, or primary care practice setting
- 5+ years of registered nursing experience in care management and/or previous value-based care leadership experiences in an ambulatory or managed care setting
- 8+ years of recent progressive healthcare experience
- Proven ability to lead, coordinate, and facilitate new ideas into action while collaborating across multiple, dynamic, projects and teams
