Job Summary
A healthcare and retail pharmacy company is filling a position for a Remote Medicare and Medicaid Care Coordinator.
Candidates will be responsible for the following:
- Identifying and escalating quality of care issues through established channels
- Coordinating and implementing assigned care plan activities and monitors care plan progress
- Identifying high risk factors and service needs
Applicants must meet the following qualifications:
- 2 years experience in behavioral health, social services or appropriate related field equivalent to program focus
- Managed Care experience preferred
- Case management and discharge planning experience preferred
- Bachelor’s degree or non-licensed master level clinician
