Job Summary

A healthcare company has an open position for a Remote Member Health Advisor.

Core Responsibilities Include:

• Conducting standardized Health Risk Appraisals
• Meeting daily production standards in terms of member outreach
• Documenting all assessments, activities and education provided

Qualifications Include:

• Completion of an accredited LVN or LPN Program
• 1-3 years in case management, disease management, managed care or medical or behavioral health settings
• License must be active, unrestricted and in good standing
• Must be licensed in CA, UT, WA, SC, MI, WI, IL, FL, MS, NM, OH
