Job Summary
A healthcare provider has an open position for a Remote Mother Nurture Project Patient Navigator in Detroit.
Must be able to:
- Identify potential clients, complete initial needs assessment, and maintain follow-up
- Provide informal counseling, education, and social support to clients
- Develop and maintain strong working relationships with external and internal contacts
Qualifications for this position include:
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required
- Community Health Worker certification preferred or obtain within 6 months of hire
- Proficient in Google Suite
