Job Summary
A healthcare company is filling a position for a Remote New York Licensed Nurse Practitioner.
Candidates will be responsible for the following:
- Conducting video visits for chronic care management and remote patient monitoring
- Creating an appropriate care plan for the member
- Conducting care coordination
Qualifications for this position include:
- Attend meetings
- Certified and licensed as a Nurse Practitioner in good standing in the state of New York
- Master’s or doctoral degree from an accredited institution for nurse practitioners
- Certification from ANCC, or equivalent, as an Adult, Family, Geriatric, and/or Acute Nurse practitioner
- 1+ years of Nurse Practitioner Experience
- 1+ years of telephonic triage or equivalent experience
