Job Summary

A healthcare company is in need of a Remote New York Licensed Nurse Practitioner.

Core Responsibilities of this position include:

• Conducting video visits for chronic care management
• Conducting care coordination
• Serving as a consulting resource on care management practice

Applicants must meet the following qualifications:

• Certified and licensed as a Nurse Practitioner in good standing in the state of New York
• Master’s or doctoral degree from an accredited institution for nurse practitioners
• Medicare participation and ability to have the company bill for services on your behalf
• Certification from ANCC (or equivalent) as an Adult, Family, Geriatric, and/or Acute Nurse practitioner
• 2+ years of clinical experience working with complex adult populations
• 1+ years of Nurse Practitioner Experience
