Job Summary
A healthcare company has an open position for a Remote Nurse Care Manager.
Must be able to:
- Deliver coordinated, patient-centered virtual Care Management by telephone and/or video
- Improve members’ health outcomes
- Generate impactful care plans together with members and our multidisciplinary care team
Position Requirements Include:
- Registered Nurse, licensed and in good standing with the nursing board of their state
- Willingness to become licensed in multiple states
- 2+ years experience working in Complex Care and Acute Case Management
- Comfortable discussing a wide variety of medical conditions
