Job Summary

A healthcare company needs applicants for an opening for a Remote Nurse Care Manager I or II.

Must be able to:

• Conduct patient assessments to identify individual needs
• Implement care plan by facilitating authorizations/referrals as appropriate
• Monitor and evaluate effectiveness of the care management plan

Required Skills:

• 3 years of case management, condition specific clinical experience, home health/discharge planning experience
• Or any combination of education and experience, which would provide an equivalent background
• Current, unrestricted RN license in applicable state(s)
• Multi-state licensure is required if this individual is providing services in multiple states
• Knowledge of medical management process required for level II
• Ability to interpret and apply member contracts required for level II
