Job Summary
A health insurance company is in need of a Remote Nurse Care Manager II.
Must be able to:
- Ensure member access to services appropriate to their health needs
- Conduct assessments to identify individual needs & a specific care management plan to address objectives & goals
- Implement care plan by facilitating authorizations/referrals as appropriate within benefits structure
Applicants must meet the following qualifications:
- 3 years of case management experience; or any combination of education and experience
- Multi-state licensure is required if this individual is providing services in multiple states
- Current, unrestricted RN license in applicable state(s) required
