Job Summary

A not-for-profit healthcare system has a current position open for a Remote Nurse Care Manager.

Core Responsibilities Include:

• Facilitating an organized, interdisciplinary team approach to care management to assure a quality, cost-efficient plan for identified patients
• Identifying gaps in care and barriers to attaining improved health
• Serving as a resource to physicians and staff, and monitor and follow-up on patient’s progress regularly

Qualifications for this position include:

• Graduate of an approved nursing program
• Bachelor’s Degree in Nursing, Health Sciences or other related field
• 3 years of nursing experience preferably in home health, ambulatory care, community public health, or case management
• Knowledge and experience teaching self-management
• Excellent telephone and interpersonal communication skills
• Strong knowledge of Medical Terminology
