Job Summary
A healthcare and retail pharmacy company is seeking a Remote Nurse Case Manager.
Must be able to:
- Implement and coordinating all case management activities
- Interact with members/clients telephonically or in person
- Assess and analyze injured, acute, or chronically ill members/clients
Qualifications for this position include:
- Ability to travel within a designated geographic area
- 3+ years clinical practical experience
- 2+ years CM, discharge planning and/or home health care coordination experience
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills
