Job Summary
A health insurance company is searching for a person to fill their position for a Remote Nurse Case Manager I in Portsmouth.
Core Responsibilities of this position include:
- Ensure member access to services appropriate to their health needs
- Conduct assessments to identify individual needs & a specific care management plan to address objectives & goals
- Implement care plan by facilitating authorizations/referrals as appropriate within benefits structure
Applicants must meet the following qualifications:
- Requires a BA/BS in a health related field
- 3 years of clinical experience. Current, unrestricted RN license in applicable state(s) required
- OR any combination of education and experience, which would provide an equivalent background
- Multi-state licensure is required if this individual is providing services in multiple states
