Job Summary

A health insurance company has a current position open for a Remote Nurse Disease Management I.

Individual must be able to fulfill the following responsibilities:

  • Conduct behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs
  • Identify and/or coordinate specific health coaching plan needs to address objectives and goals identified during assessments
  • Interface with provider and other health professionals to coordinate health coaching plan for the member

Qualifications Include:

  • 2+ years of condition specific clinical or home health/discharge planning experience
  • AS in nursing; or any combination of education and experience which would provide an equivalent background
  • Current unrestricted RN license in applicable state(s)
