Job Summary

A healthcare staffing and recruiting firm is seeking a Remote Nurse or Social Worker or Counselor Field Case Manager.

Core Responsibilities Include:

• Playing an integral part in the coordination of care assessing the member’s condition
• Coordinating and monitoring the care of Health Plan members and developing a nursing plan of care
• Participating in interdisciplinary conferences

Required Skills:

• Must be willing and able to travel in area with mileage reimbursed: 30-50%
• Valid, unrestricted state Registered Nurses (R.N.) license
• Bachelor's degree in behavioral health or human services
• At least 2 years experience in behavioral health, social services or appropriate related field equivalent to program focus
• Computer literacy (MS Office) and typing skills are necessary
• Strong oral and written communication skills
