Job Summary
A company is looking for a Multi-State Remote Nurse Practitioner for a contracted position in IN & IA.
Key Responsibilities:
- Provide video-based care to patients in a remote care program
- Deliver empathic care to members and assess their competencies
- Determine members' eligibility for the clinical program and handle clinical escalations
Required Qualifications:
- Hold an unencumbered, active state license; multi-state licenses preferred
- Minimum 3+ years experience managing patients with diabetes
- Bachelor of Science degree in Nursing (BSN) & Master of Science in Nursing (MSN)
- Spanish-speaking preferred, certification required
