Job Summary

A healthcare company is filling a position for a Remote Patient Care Navigator.

Individual must be able to fulfill the following responsibilities:

  • Assisting patients with the navigation of health care services and systems
  • Removing barriers not limited to care coordination, chronic disease outreach and post visit primary care outreach
  • Performing outreach to patients via phone and in-app messaging channels

Position Requirements Include:

  • At least 2-3 years of Patient Navigation, Community Outreach Worker or healthcare experience
  • Knowledge of primary care services, electronic health records and practice management software is a plus
  • A strong believer that patients should have greater access to their healthcare team, to their own health information, and a greater ability to make informed decisions about their care
  • A caring individual with true compassion, creativity and flexibility
  • Excited to build a new and exciting model for healthcare delivery
  • Excellent at building successful and respectful relationships with the entire team
