Job Summary
A healthcare company is filling a position for a Remote Patient Care Navigator.
Individual must be able to fulfill the following responsibilities:
- Assisting patients with the navigation of health care services and systems
- Removing barriers not limited to care coordination, chronic disease outreach and post visit primary care outreach
- Performing outreach to patients via phone and in-app messaging channels
Position Requirements Include:
- At least 2-3 years of Patient Navigation, Community Outreach Worker or healthcare experience
- Knowledge of primary care services, electronic health records and practice management software is a plus
- A strong believer that patients should have greater access to their healthcare team, to their own health information, and a greater ability to make informed decisions about their care
- A caring individual with true compassion, creativity and flexibility
- Excited to build a new and exciting model for healthcare delivery
- Excellent at building successful and respectful relationships with the entire team
