Job Summary
A health insurance company needs applicants for an opening for a Remote Patient Centered Care Consultant.
Individual must be able to fulfill the following responsibilities:
- Obtain and analyze quality metrics and reports for care opportunities
- Support practice implementation of care coordination and care management
- Identify action plans for providers to implement to improve cost, quality and the patient experience
Skills and Requirements Include:
- BA/BS, 3 years health care experience; or any combination of education and experience
- Ability to participate in developing program recommendations and action plans
- Ability to support the efforts of providers and practice teams
