Remote Patient Monitoring Team Lead

Job Summary

Serving as the escalation point for the Data Quality Reviewers, the full-time Remote Patient Monitoring Team Lead will manage daily remote patient monitoring tasks, generate monthly summary reports, and facilitate communication between teams while working remotely.

Key responsibilities

• Identifying medical changes or abnormalities through daily monitoring for escalation and clinical review
• Generating and reviewing monthly summary reports on patients' health conditions for treatment plan recommendations
• Adhering to established policies and protocols while developing relationships to enhance program communication

Required qualifications

• Some college or technical education; an associate degree or higher preferred
• Minimum of 3 years of health field-related work or volunteer experience
• Proficiency in navigating electronic health records
• Proficient with Microsoft Office Suite
• Ability to understand and interpret basic math logic such as averages and graphs
