Job Summary

A healthcare company has a current position open for a Remote Personal Health Nurse Case Manager.

Individual must be able to fulfill the following responsibilities:

  • Review daily calendar for new episodes and tasks due
  • Manage accounts using the 4-Pringes approach
  • Engage the PCP and treating physicians in the personal health management process

Must meet the following requirements for consideration:

  • Ability to travel
  • Five (5) years of UM/CM experience
  • Bachelor degree in a health related field
  • Three to five (3-5) years of diverse clinical experience required
  • RN (Registered Nurse) license required
  • Current active professional license in the state of residence
  • Eligibility for unrestricted professional licenses in all states
